JOB SUMMARY:
As a Registered Nurse, the post holder is required to adhere to NMC standards and guidelines of practice, education and conduct.
The post holder has responsibility for the screening and assessing of people for NHS Continuing Health Care (CHC) and for determining the Registered Nurse Care Contributions (RNCC) for all people requiring care in a Care Home. This may include people currently in hospital who are being discharged to a Care Home with Nursing in another area. The post holder will also undertake reviews for people in receipt of NHS Continuing Health Care and RNCC determinations as appropriate.
The post holder will be expected to liaise with and develop partnerships with Social Service providers and other relevant agencies.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
1. Clinical leadership
a) To identify health needs by undertaking an assessment or review in partnership with individuals in order to inform the NHS CHC and RNCC processes.
b) To act as an effective member of the Funded Nursing Care Team (FNCT), participating in induction and orientation programmes as required.
c) To participate in the Trust’s supervision strategy including the provision of supervision as required.
d) To be aware of and assist in effective budget management.
e) To act as an information source on the Continuing Health Care and RNCC processes, including the provision of support and advice.
2. Practice performance and service delivery.
a) Alongside the specialist assessment nurse, specialist support nurse and manager for commissioning for individual patients participate in the process of screening and assessment for CHC, including review of all people currently in receipt of CHC funding. This may include attending CHC panel meetings either presenting applications and/or participating in decisions.
b) To undertake to determine and review RNCCs for residents in, or planning to enter, a Care Home with Nursing.
c) To keep accurate, contemporaneous documentation and written reports as required.
d) To accept new referrals and explore, with appropriate agencies, possible alternatives to long term nursing care.
e) To attend and participate in meeting/panels/case conferences/discussions as appropriate.
f) To report relevant information to colleagues and team leaders as required – including peer reviews.
g) To record activity levels and outcomes to meet contractual requirements.
h) To maintain accurate computerised records.
i) To report clinical and non-clinical concerns and risk assessment issues to the FNCT team leader.
j) To participate in and develop audit programmes to measure the effectiveness and quality of the service.
3. Education, training and service development
Nursing and Midwifery Council.
Registered Nurses are seen as being professionally accountable and shall adhere to the NMC Code of Professional Conduct for the Nurse, Midwife and Health Visitor.
a) To participate in education programmes as necessary.
b) To keep up-dated with local and national criteria pertaining to the role.
4. Health promotion and public health
a) To provide health advice to Care Home with Nursing staff including suggestions as to other agencies who may offer appropriate services.
b) To carry out any other duties in line with the grade and scope of the post.
The work is Mon-Fri 09.00-17.00, car drivers are not essential as the work will be centre based.

JOB SUMMARY:
As a Registered Nurse, the post holder is required to adhere to NMC standards and guidelines of practice, education and conduct.
The post holder has responsibility for the screening and assessing of people for NHS Continuing Health Care (CHC) and for determining the Registered Nurse Care Contributions (RNCC) for all people requiring care in a Care Home with Nursing within Brighton and Hove. This may include people currently in hospital in Brighton and Hove who are being discharged to a Care Home with Nursing in another area. The post holder will also undertake reviews for people in receipt of NHS Continuing Health Care and RNCC determinations as appropriate.
The post holder will be expected to liaise with and develop partnerships with Social Service providers and other relevant agencies.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
1. Clinical leadership
a) To identify health needs by undertaking an assessment or review in partnership with individuals in order to inform the NHS CHC and RNCC processes.
b) To act as an effective member of the Funded Nursing Care Team (FNCT), participating in induction and orientation programmes as required.
c) To participate in the Trust’s supervision strategy including the provision of supervision as required.
d) To be aware of and assist in effective budget management.
e) To act as an information source on the Continuing Health Care and RNCC processes, including the provision of support and advice.
2. Practice performance and service delivery.
a) Alongside the specialist assessment nurse, specialist support nurse and manager for commissioning for individual patients participate in the process of screening and assessment for CHC, including review of all people currently in receipt of CHC funding. This may include attending CHC panel meetings either presenting applications and/or participating in decisions.
b) To undertake to determine and review RNCCs for residents in, or planning to enter, a Care Home with Nursing.
c) To keep accurate, contemporaneous documentation and written reports as required.
d) To accept new referrals and explore, with appropriate agencies, possible alternatives to long term nursing care.
e) To attend and participate in meeting/panels/case conferences/discussions as appropriate.
f) To report relevant information to colleagues and team leaders as required – including peer reviews.
g) To record activity levels and outcomes to meet contractual requirements.
h) To maintain accurate computerised records.
i) To report clinical and non-clinical concerns and risk assessment issues to the FNCT team leader.
j) To participate in and develop audit programmes to measure the effectiveness and quality of the service.
3. Education, training and service development
Nursing and Midwifery Council.
Registered Nurses are seen as being professionally accountable and shall adhere to the NMC Code of Professional Conduct for the Nurse, Midwife and Health Visitor.
a) To participate in education programmes as necessary.
b) To keep up-dated with local and national criteria pertaining to the role.
4. Health promotion and public health
a) To provide health advice to Care Home with Nursing staff including suggestions as to other agencies who may offer appropriate services.
b) To carry out any other duties in line with the grade and scope of the post.
