My client is looking for an expereinced Qualified Social Worker to work on a Hospital Discharge Team based in Peterborough.
The role will require the successful candidate to:
• Undertake Assessment of Needs
• Mental Capacity Assessments
• Continuing Care Assessments
• Ensuring that Adult Protection Investigations are undertaken in a timely manner in order to keep vulnerable adults safe
• Liaising with all professionals as part of multi agency working
• Undertake reviews of services provided
• IT skills used for keeping all records updated daily
Funding has been granted for an initial period of 3 months but will more than likely be extended.
Ideally candidates will have a minimum of 2-3 years direct working experience/knowledge within an Adults setting, preferably a Hospital setting. All applicants will need to hold a Social Work certification, such as DipSW, General Social Care Council registration (GSCC), a current clear CRB issued within the last 12 months and hold a valid UK driving license.
