Duties will include:
A proactive can do attitude, comfortable with dealing with challenging behaviours
Visual presence throughout the shift and offering support to clients
Record keeping of daily activities/observations of clients
Provide detailed handovers daily
Support in accessing information, i.e benefit system, crisis loans, GPs etc
Completing risk assessments
Environmental safety checks
Candidates must also have the following:
12 Months UK care experience within the last 2 years
At least 5 year checkable work history
Original documents i.e. passport, valid national insurance number
Minimum NVQ 2 or 3 qualified or equivalent qualification
You must be willing to complete a new advanced CRB with Metrocare
You must be flexible, and willing to travel.

Working within the clients home
Outside in the local area
On the wards of the local hospital
In community or day centres
Outreach workers provide help and relieve stress for persons with enduring mental health problems and are responsible to the Senior Outreach Worker, Community Support Services Manager or Community Mental Health Officer.
Outreach workers will

Monitor and oversee any medication prescribed by qualified medical practitioners and check for any changes that have been made in dosage
Help the client with preparation of meals, shopping, cleaning and laundry
Note any significant changes in clients health, outlook and behaviour and refer upwards for advice about what action to take
Report to the senior outreach worker/manager about any alteration in the clients family circumstances
