Brook Street Social Care are seeking a dedicated and compassionate Project Worker to join our client's Supported Living Service in Lancaster on a temporary-to-permanent basis.
This is a rewarding opportunity to support adults with complex mental health needs, helping them to achieve greater independence, stability, and improved wellbeing. The successful candidate will work closely with individuals experiencing a range of challenges, including mental health conditions, homelessness, substance misuse, housing instability, and social exclusion.
As a Key Worker, you will build positive, professional relationships with residents, providing person-centred support and empowering individuals to work towards their personal goals.
Key Responsibilities- Manage a caseload of residents and act as their designated Key Worker.
- Develop, implement, and review person-centred support plans and risk assessments.
- Support residents with housing applications.
- Assist individuals in accessing mental health services, healthcare, benefits, housing providers, and other community resources.
- Promote recovery, independence, and social inclusion.
- Work collaboratively with external agencies, including mental health teams, local authorities, housing providers, probation services, and substance misuse services.
- Maintain accurate records, case notes, and outcome-focused documentation.
- Safeguard vulnerable adults and contribute to effective risk management practices.
We are looking for someone who is passionate about supporting people with complex needs and committed to delivering high-quality, person-centred support.
You will ideally have:
- Experience working within supported housing, homelessness services, mental health services, substance misuse services, or a related field.
- Knowledge of recovery-focussed and trauma-informed approaches.
- Experience managing challenging situations professionally and calmly.
- Strong communication, organisation, and interpersonal skills.
- The ability to work independently and as part of a multidisciplinary team.
- A good understanding of safeguarding and professional boundaries.
- A commitment to equality, diversity, and inclusion.
A relevant qualification in Health and Social Care, Mental Health, Housing, or a related discipline is desirable but not essential.
